The financial part of your health coverage is important, too.
The 1095 form shows the IRS information about your health plan for the year.
If you’re enrolled in a Health Insurance Marketplace plan and got financial help from the government to lower your monthly cost for insurance (also called a subsidy or an advanced premium tax credit), you’ll need to reference IRS form 1095-A to file your Premium Tax Credit form (Form 8962). The Marketplace should send you Form 1095-A every year, or you can log in to your Marketplace account at HealthCare.gov and download it.
