Where can I find my yearly health coverage tax form?

The 1095 form shows the IRS information about your health plan for the year.

  • If you’re enrolled in a Health Insurance Marketplace plan and got financial help from the government to lower your monthly cost for insurance (also called a subsidy or an advanced premium tax credit), you’ll need to reference IRS form 1095-A to file your Premium Tax Credit form (Form 8962). The Marketplace should send you Form 1095-A every year, or you can log in to your Marketplace account at HealthCare.gov and download it.

  • If you have group coverage through your employer, please contact your employer's HR department for your IRS Form 1095-C.
  • The IRS requires Florida Blue to file a Form 1095-B for our members with Affordable Care Act health plans as proof of minimum essential coverage (MEC).

Log in to your member account to download a copy of Form 1095-B or request a printed version be mailed to your home address.
